ABSTRACT

Objective: [1] Assess the knowledge towards the various aspects of the HPV, cervical cancer and the HPV vaccine. [2] Determine the percentage of participants who have received the vaccine. [3] Determine the attitude and behaviour -intent of participants regarding cervical cancer and the HPV vaccine


Materials and methods: A cross-sectional study was conducted amongst female university students in Ajman, UAE. Participants aged >18 years completed a self-administered validated, pilot-tested questionnaire. The knowledge was assessed in four domains: general knowledge, knowledge on prevalence and etiology, prevention, and risk factors. A scoring system was used and participants were identified to have low scores, if they scored < 50% of the corresponding total score. The SPSS version 21 was used to analyze the data, X2 test and Fisher's test were used to test the significance of association. A p value of <0.05 was accepted as significant


Results: The study included 226 participants who were mostly >/=20 years of age [57%], Non-Arabs [67%], medical students [52%], first year university students [33%], single [96%], without health insurance [52%], and having stable financial status [58%]. The proportion of participants who scored low for general knowledge, prevalence and etiology, prevention and risk factors were 63%, 59%, 79% and 98% respectively. Low knowledge score was significantly associated with age, for the general knowledge [P=0.04] and the knowledge on prevalence and etiology [P=0.001]. Junior in comparison to senior student had significantly higher proportion of low scores for the general knowledge [P<0.001], knowledge on prevalence and etiology [P=0.001], and for Prevention [P=0.02]. The non-medical students had significantly higher proportion of low scores for the general knowledge [P=0.04] and prevalence and etiology [P<0.01] compared to medical students. Receipt of HPV vaccine was only reported by 16 participants [7.3%] and the majority of the respondents [72%] reported their willingness to take the vaccine. Positive attitude was found in 67% of respondents


Conclusion The prevalence of low knowledge is high. The main gap in knowledge was for the risk factors for cervical cancer. Majority of students have positive attitude towards the vaccine and its usage

ABSTRACT

Germ cell tumors are rare in children. This group of tumors in children is distinct from that in adults in terms of their behavior, histological nature, metastasis and the treatment needed. 85% of the yold sac tumors in children present as clinical stage I disease as compared to 35% in adults. AFP can be used as reliable tumor marker as the levels are increased in more than 90% of yolk sac tumors. In children inguinal orchidectomy followed by stringent surveillance for two years is the preferred treatment option. Combination chemotherapy may be reserved for metastatic disease or recurrence. Retroperitoneal Lymph Node Dissection [RPLND] is preferable in adults. In this case report we describe the clinical presentation, imaging findings, diagnosis and up-to-date management of yolk sac tumor in an eight-month old child

ABSTRACT

A case of ectopic salivary gland tissue in a routine tonsillectomy specimen is reported and the literature is reviewed. Tonsillectomy specimens are routinely sent for histopathologic evaluation to assess the nature of inflammatory process as well as to exclude occult malignancy. A tonsillectomy specimen from a young woman who underwent surgery for recurrent attacks of tonsillitis in the previous six months was received at the histopathology laboratory of Gulf Medical College Hospital, Ajman. Routine microscopic examination revealed tonsilar tissue with chronic inflammation and lobules of mucous secreting salivary acini with ducts adjacent to the surface squamous epithelium of the tonsilar tissue

ABSTRACT

Corpus cancer is the most frequently occurring female genital cancer. Clear cell carcinoma is a rare form of endometrial cancer accounting for 1-1.5% of all endometrial carcinomas and is often associated with an aggressive clinical behaviour and poor clinical outcome. The molecular pathways involved in development of clear cell carcinoma are still unclear with the present presumption that it arises from atrophic endometrium. Patients with clear cell carcinoma often experience relapse in pelvis, in para-aortic nodes and at distant sites. Total abdominal hysterectomy and bilateral salphingo-oopharectomy with comprehensive surgical staging is the standard surgical treatment of patients with clear cell carcinoma of endometrium, whereas pelvic irradiation, para-aortic irradiation, whole abdominal irradiation and chemotherapy have been widely employed as post operative therapy. This is a case report of a rare case of clear cell carcinoma of endometrium with adenomyosis. A 65 years menopausal lady presented with bleeding per vagina for the past 6 months. She was married and nulliparous. She was a hypertensive patient on irregular treatment. Physical examination was unremarkable. She was evaluated for her post menopausal bleeding. She had an unsatisfactory colposcopy. MRI showed a large polypoidal endometrial mass lesion with an impression of neoplastic endometrial growth. With a provisional diagnosis of endometrial polyp/ carcinoma endometrium she was planned for total abdominal hysterectomy and bilateral salphing-oopharectomy surgery. Following a difficult surgery for a suspected uterine malignancy she had a good post operative recovery. Histopathological report revealed a high grade endometrial clear cell carcinoma with focal papillary serous differentiation with adenomyosis and leiomyoma. In view of high grade invasive carcinoma of endometrium she was referred to a radiotherapy unit for post operative radiotherapy. This case was analyzed with review of other reported case. Review re-emphasised the need of accurate histopathological diagnosis in addition to clinical staging especially in rare variants of endometrial carcinomas like clear cell carcinoma and squamous carcinoma in optimizing the treatment. However the importance of myometrial invasion cannot be overlooked especially when adenomyosis co-exists with adenocarcinoma. It is important to have a clear distinction between a true myometrial invasion by adenocarcinoma and involment of adenomyotic foci by malignant cells without myometrail invasion in determining the staging and prognosis. In view of non-availability of commonly accepted guidelines for the management of patients with clear cell carcinoma of endometrium, and adequate molecular, characterization of clear cell carcinoma is strongly warranted in order to identify new biological prognostic variables of the disease and to develop novel molecular targeted therapies
